Case Scenario: A 14-year-old boy fromMumbaiwho was diagnosed with Noonan's syndrome was admitted at MIOT Hospitals,Chennaion July 14, 2012. Noonan's syndrome is an autosomal congenital disorder considered to be a type of dwarfism. The features include narrowing of heart valve, hole in the heart, depressed breast bone (pectus excavatum), webbed neck and a flat nasal bridge. It occurs 1 in 1000 child births. This boy was diagnosed to have a large hole in the heart (ASD) as a child, when was 4 years old. He was having progressive breathlessness and recently when he went for the treatment for the hole in the heart, he was found to have a large aneurysm of the aorta with leak in the aortic valve. The local doctors fromMumbaitold him that it is extremely difficult to do the operation and he was advised to go abroad to get it done. This combination of aneurysm of the aorta with leak in the aortic valve and a hole in the heart has not been reported before. The young boy in addition had a chest wall abnormality called pectus excavatum in which the breast bone has gone very deep and this has pushed the heart completely to the left side of the chest. Recently, he was having breathlessness and was not able to sleep properly and was referred to MIOT Hospitals for surgery since it could not be done anywhere else. Normally, open heart surgery is performed by cutting the breast bone because the heart is pushed behind. Done this way, the operation is easy to perform. But in case of this young boy, since the heart was totally on the left side of the chest, it was impossible to do this operation through the midline. The team of cardio thoracic surgeons at MIOT Hospitals searched extensively in the literature to find out which is the best way of doing this operation, but could not find a single case report of this combination of disease. Surgery Procedure: The young boy underwent open heart surgery onJuly 16, 2012. MIOT Hospitals performed this operation by opening the left side of the chest. To see the heart and do the operation with a chest wall abnormality through left side of the chest was extremely very difficult. The patient was connected to a heart lung machine by making holes in the groin arteries. After this, he was cooled and the heart was stopped andthe replacement of the aortic valve and aortic root (Bentall's operation) with re-implantation of the coronary arteries was done. After that, the hole in the heart was closed. Post-sur er ,the oun bo was ivens ecialnursin carefor imrovin his
lung function because the heart was occupying the whole left side of the chest pressing the left lung. He made an uneventful recovery and will be discharged onAugust 01, 2012. "To our knowledge doing a Bentall's operation and closing the hole (ASD) by opening the left side of the chest in this type of congenital abnormality in a child has not been reported in the world before," said Dr. V.V. Bashi, Chairman, Centre for Thoracic and Cardio Vascular Care, MIOT Hospitals. The surgical team was headed by Dr. V.V. Bashi assisted by Dr. Vijith K Cherian, Dr. Kannan R Nair, Dr. Harilal and Dr. Gopal. Anaesthetic team was headed by Dr. Aju Jacob and assisted by Dr. Jyotsana and Dr. Ajith.
