MER Recorder User s Guide Medical Event Report (MER) Recorder s Guide Login as a MER Recorder To login: 1. In theNaval Disease Reporting System internet (NDRSi)page, enter your Login ID and Password.
Note: Remember that your password is case sensitive, is 8-12 characters long, and includes an upper case character, a lower case character, a number and a non-alpha character (i.e., !, @, #, $, etc.).

Note: NDRSi accounts for MER Recorders must be created by an Administrator of the database. If this is your first time logging into the NDRSi Database, seeCreating Your Profile. Otherwise, the MER Recorder Main Page will open (seeMER Recorder Features).

Creating A Profile 1. In theProfile Pagethat opens, change your Profile information, which includes:
 
Click the “Save” icon to save changes to your profile.
Selecting a Question from the drop-down menu and answering it with a memorable one-word answer is required.
 Changing your password (optional). If you change your password, remember it must contain: ƒ an upper case letter (A, B, C, etc.) ƒ a lower case letter (a, b, c, etc.) ƒ a number (1, 2, 3, etc.) ƒ a non-alpha character (!, @, #, etc.) ƒ at least 8 characters  no more than 12 characters ƒ
 Selecting a question from the drop-down menu, and entering a one-word, memorable answer you will be able to use in case you forget your password (requiredbefore you can proceed).
 Entering your profile information, including: your first and last name, UIC/RUC, phone number, and e-mail address. Note: Changing your Password is optional, but before you are able to proceed to the MER Recorder Main Page,you must set a question and answer and complete the profile section question and answer will. The be used in case you forget your password.
2. Click to finish changing your profile.  Manage Sponsor/FMP Profiles– This feature allows MER Recorders tManage Sponsor/ FMP Profiles. Enter/Edit Medical Event Report(s) by SSN
In theMER Recorder Main Page ., click 1. In theEnter/Edit Medical Events Reports by SSNpage, enter the SSN of the Sponsor or Dependent you would like to work with.

4. If the SSN is correct, new controls will appear to work with the identified Sponsor account.
 In the “Select the FMP code associated with this Sponsor’s account” drop-down list, the Sponsor’s account (FMP code 20) is selected by default. Any Medical Event Reports reported for this Sponsor will appear in the “List of Previously Filed Medical Event Reports for this Patient” tableat the bottom of the page. 5. To review Medical Event Reports for the FMP associated with the Sponsor, click on the “Select the FMP code associated with this Sponsor’s account:” drop-down list. All of the FMPs associated with this Sponsor are contained within the list. Selecting an FMP from the drop-down list will populate the “List of Previously Filed Medical Event Reports for this Patient” table with the Medical Events Report filed for this FMP. 6. selected patient, they will appear in theIf Medical Event Reports already exist for the table at the bottom of the page. If no Medical Event Reports exist for the selected patient, seeCreate a Medical Event Report “List of Previously Filed Medical Event. The Reports for this Patient” table canbe viewed in different ways: a.  sort Tois sorted by “Date of Onset” by default.The Medical Event Reports table Medical Event Reports by another subject, move the mouse over the list of column titles. The table can be sorted by any column where the table column heading switches to underlined blue text when the mouse moves over it. For example: to sort the cases by Case Status, click on the table column heading “Case Status.” Cases will be arranged alphabetically (i.e., Confirmed, Not Confirmed, and Pending). The A-Z icon will appear next to Case Status” title, indicating that the table is ordered alphabetically according to this field. Click on “Case Status” again and the table will be displayed in reverse order (i.e., Pending, Not Confirmed, Confirmed), and the sort order icon will be reversed, from Z-A. b.  is located next to theMedical Event Reports, use the funnel icon thatTo filter the column headings. A filter can be created based on any column heading that has the

MER Recorder User s Guide funnel icon next to it. For example, to display the selected reports of a patient based on an ICD9 Code, click the funnel icon, which is next to the “ICD9CODE” column header. This will bring up a dialog box which contains a drop-down list. The drop-down list only contains a selection of ICD9 Codes that have been previously filed for the current patient.
 Click on the drop-down list and select one of the ICD9 Codes from the list. This will filter the current patient’s reports, displaying only the reports of the selected ICD9 Code.
 The funnel will appear outlined in red, indicating that the patient’s medical reports are being filtered on an ICD9 Code. To reset the display and view all of the patient’s reports again, click the funnel icon, and from the drop-down list select “**View All”. Once the “**View All” has been selected, the funnel icon will no longer be highlighted in red, indicating that the filter is not on and all files are shown. 7. To edit MER Cases that have already been filed, seeEdit a Medical Event Record. If there are no Medical Event Reports filed for this Sponsor, a message will appear, “No cases have been entered for this individual.” To file a new Medical Event Report see Create a Medical Event Record. Create a Medical Event Report After identifying the Sponsor or FMP account (seeEnter/Edit Medical Event Report(s) by SSN), create a new Medical Event Report by:

 2. In theMedical Event Recordpage that opens, the Sponsor’s Demographic section is filled by default from the Sponsor’s or FMP’s Profile. 3. To update a Sponsor or FMP’s demographic information, seeManage Sponsor/FMP Profile). 4. Click on the Medical Event section “Diagnosis (ICD-9 code)” drop-down list to see a complete list of ICD9 Codes in alphabetical order. Select the correct ICD9 Code for the Medical Event Report being reported. Selecting an ICD9 Code will change the Medical Event form being displayed. The patient demographic section will not change.

Edit a Medical Event Report After identifying the Sponsor or FMP account (seeEnter/Edit Medical Event Report(s) by SSN), review and/or update an already existing Medical Event Report for a patient by finding the report you would like to review in the “List of Previously Filed Medical Event Reports” table at the bottom of the Manage Medical Event Reports by SSN page.

MER Recorder User s Guide 2. Click . 3. If the SSN you entered does not exist in the NDRSi database, the following message will appear: The SSN was not found in the NDRSi database. Select “Register New Sponsor” to create an account for this Sponsor in the NDRSi database. A Sponsor profile must be created before submitting a MER for the Sponsor or a Dependent.  If this message does not appear, the SSN is already registered as in the NDRSi database. SeeCreate a Medical Event Report.  
 
 4. If the Sponsor does not already have an account, click the “Register New Sponsor” button. 5. sor Profile Pagethat opens, fill in the fields as completely as possible. Click . 6. If any of the fields have not been filled correctly, a message will appear, “Please make sure all of the required fields are filled.”  There will also be red asterisks next to the fields that are invalid. Correctly fill in any fields that are invalid and click the “Submit” button again. 7. When the account has been added to the database, the following message will appear: This Sponsor’s Profile has been saved. Click “Close” toreturn to the Manage Medical Event by SSN page.
A pop-up window with a Right to Privacy statement will appear. Click . If you need to create a FMP account seeManage FMP Profiles. 8. Click .
