catholicassesments.com Directions for Administering the Faith Knowledge Assessment In the test booklets printed in or before the 2003-4 school year, students are to complete only the first section on Faith Knowledge. Please check to see that the students are using the test level appropriate for their grade. The student answer sheet for the Faith Knowledge assessment is a tailored version of the answer sheet labeled “Exam System II.” This tailored version has “Catholic Assessments” printed over the section in which students enter their last names. BE SURE ALL OF THE STUDENTS ARE USING THE TAILORED VERSION. Pass out the test booklets first. Ask the students to leave them closed on their desks. Then pass out the answer sheets. As the students begin to fill out their answer sheets, ask them to do the following: Turn to side that reads “ General Purpose Answer Sheet” and read the “Marking Instructions.” Ask the students to write in below the box with marking instructions their school or parish name and the test form as it appears on the cover of their test. Then ask them to turn to the side that says “EXAM SYSTEM II.” Ask them to gird in their last name and first name (as space permits). Then ask them to grid in their gender, grade level, date of birth. Only columns K and N and O in this section will be used for the present. In column K, ask them to identify themselves as either Catholic or non-Catholic. They should grid in #1 if they are Catholic and #2 if they are not. In column N, ask them to identify how many years they been attending this school or parish program as follows:  Ifthis is your first year, please mark #1  Ifthis is your second or third year, please mark #2  Ifthis is your fourth or fifth year, please mark #3  Ifthis is your sixth, seventh, or eighth year, please mark #4  Ifyou have been in the program nine or more years, please mark #5
In column O, ask them to identify the language most often spoken at home as follows:  IfEnglish is used, please mark #1.  IfSpanish or Portuguese is used, please mark #2  Ifan Asian language is used, please mark #3.  Ifsome other language is used, please mark #4 Thank the students for their cooperation with this section. Ask them to open their test booklets and read the directions.Please advise them that they haveapproximately45 minutes to complete the Faith Knowledge questions in the test booklet. Ask them to begin. For grade three students, you may choose to read aloudto the entire group or class the directions and the questions and answer options. Those students for whom special accommodations are made on other standardized tests should benefit by the same accommodations on this assessment. The Header Sheet Please complete a STEWARD Header Sheet for each batch of answer sheets for a grade level group returned for scoring. If you have two small classes (say twelve or less) at one grade level, you would be advised to combine them as one class. Your print reports work better for groups of 20 or more. Your diocesan office should provide you with the information needed in the final three items. The first calls for an “Assigned Diocesan Number.”The other two are diocesan wide standards.If the diocese has already sent in this information, you need not complete these two items. Please return your completed answer sheets with their header sheet to your diocesan office following the timelines your diocese provided. Internet Reporting All Internet reporting on <catholicassessments.com> is password protected. To obtain your password, simply go the web-site and click on “For Participating Educators” and proceed as directed. With your password, you can check on the status of the processing of results for your diocese and access your own local reports.When this process is completed, your local results should be available on the web-site, provided your answer sheets were sent in to your diocesan office in time to be included with the main set of diocesan results. If your results were sent in later, you will need to check every days, after allowing some 10 days for processing. Internet reporting will give you information not available with the printed reports and will enable you to print out a separate student/parent report for each student.
