Description

There are many standard text books of surgery covering variousaspects of surgery nd their sub-specialties.
The purpose of this book is to assist students especially junior onces to acquire the necessary skills in history taken, physical examination, important investigations and the management of surgical patients.
The methodology used was clinical history case demonstration and physical examination, summary of the important clinical investigations and their results, treatment offered and their outcome. Thereafter the aethopathology of the disease condition (where necessary), some modern investitagions and their likely outcome, current treatment options available, complications of the disease condition and their likely differential diagnosis were discussed in outline fashion.
Repetations in some of the topics is deliberate and inevitable. This is to give the student deep and reflective insight into the disease conditions, their mode of presentation and possible current management options available.
Some of the chapters have short vignettes which is meant to be brain teasers for both undergraduates and post graduate students preparing for the surgical medical exams such as objective structured clinical examination ((OSCE), medical viva, written examination and clinical examination.
It is hoped to serve as a fast and focused revision guide to students of surgery and would be of immense assistance to students during ward round, ground-round and tutorials. It will improve their skill in case presentation, and in answering questions clearly, precisely and in unambiquous manner during their period of apprenticeship.
It is therefore not a replacement for standard text books of surgery and will only be useful to those who have read through standard text books of surgery.

PREFACE
There are many standard text books covering various aspect of the principles and practice of surgery and their sub-specialties.
The purpose of this book is to assist students both undergraduates and junior residents who are preparing for both the written, and oral examination to reverse easily, clearly and quickly.
It will help the student in recapitulation and remembering salient points after reading a standard text book of surgery and will serve as a revision guide for both undergraduate and junior resident doctors.
The cases presented in this book were those actually seen and managed by the author in his years of practice as a general surgeon in a semi-urban hospital in the Niger Delta region, where resources both materially and finanacially were not readily available to evaluate patient fully and comprehensibly. The doctor had to take decision based on his experience and had to dwell with patients who are ignorant and difficult to manage.
It is therefore not a replacement for standard text book of surgery which are already in circulation.
It is my hope that the users of this book will find it useful in gathering the silaint point in history taken which will assist them in passing examination in both the objective structured clinical examination (OSCE), multiple choice questions (MCQs) and will improve their spectrum in essay writing and clinical examination.
I welcome useful, valuable, constructive criticism and suggestions that would improve subsequent edition of this book.

FORWARD
With great pleasure I introduce the first edition of ‘Case Illustrations and Discussions for Undergraduates’ in Surgery, for medical students and junior residents.
This book, no doubt centres on our deep culture of medical education for the above- mentioned targets amongst others.
This maiden edition has 41 chapters laced with interesting case illustrations and discussions in our environment. Some chapters have short vignettes which are also brain teasers for both undergraduates and post graduate students preparing for the surgical medical exams such as objective structured clinical examination ((OSCE), medical viva etc.
Excitingly, some areas dealt with minimal access surgery or laparoscopic surgery as an approach to its management. This relatively ‘new’ area of minimal invasive surgery coincidentally, is my core areas of focus and one of my skill sets. A few personal inputs came in handy. The scope also covers General Surgery sub-specialties like Trauma, Urological Surgery, Pediatrics Surgery, Plastic and Hands Surgery, Trauma, and Vascular surgery. Evidence based medicine has been incorporated into each chapters.
History taking and focused examinations were copiously exhibited which will be of immense benefits, especially to the medical students. Regrettably, the lack of advance imaging such as CT or MRI in our resource constrained environment, has denied us the opportunity to enrich your knowledge and attendant input in this beautiful piece of work. Nonetheless, it’s my firm belief, that subsequent editions will have these inputs in no distant time.
The author, Dr. Benjamin I. Tabowei, is a brilliant General Surgeon and administrator per excellence, who has held several administrative positions in the past. I had the privilege to have worked under both him and Dr. Uriel Dambo, as a junior Medical Officer immediately after my National Youth Service in 2004 at the then General Hospital (now Niger Delta University Teaching Hospital), Okolobiri, where I learnt a lot of surgical trade from them and the surgical unit. I am aware that a lot of other doctors have also passed through his tutelage.
He is therefore eminently qualified to write this book having being very active in this business of surgical practice and medical education in the past three decades.
I extend my gratitude to my teachers, medical elders, colleagues, registrars and students who contributed in one way or the other to the emergence of this book. I do hope that this Case Illustration and Discussions for Undergraduate in Surgery would be utilized for its desired purpose.

CHAPTER 1
Burns
Case 1.1
B.O is a 36 year old house wife who was rushed to our facility with a two hour history of chest and abdominal pain, difficulty in breathing, and cough.
She was cooking with a stove in her kitchen which exploded and sprayed kerosene on her body while she was attempting to top the kerosene on the burning stove.
Her entire clothes had caught fire and she ran out of the kitchen, feel on the ground, rolled a few times before the fire was finally extinguished by her neighbours and relations using water soaked with some detergent.
The flames must have lasted over a minute on her body before it was estinquished and was rescued by neighbours and well wishers.
She now was having sharp and burning pains all over her body especially on her face, abdomen and anterior chest wall. Pain was aggravated by breathing, slight movement and cough.
She was now having tightness on her chest and difficulty with breathing and had developed cough which was unproductive of sputum. She feels weak, nauseated but had not vomiting.
She was not feeling thirsty but had not passed urine since the incident took place.
She admitted to have being an asthmatic patient on treatment (salbutamol inhaler and sometimes prednisolone tablets). Attack was usually precipitated by
