Medical students and doctors in training are expected to travel to different hospitals and community health centres as part of their education. Many books are too large to carry around, but the information they contain is often vital for the basic understanding of disease processes.
The Pocket Essentials series is designed to provide portable, pocket-sized companions for students and junior doctors. They are most useful for clinical practice, whether in hospital or the community, and for exam revision.

Preface to Third Edition
This book has been written primarily with the needs of medical students in mind. It aims to outline the essentials of psychiatry at the level of the final MB examination and should prove useful both as a book that can be carried by the student during the clinical psychiatric attachment and also as a revision text in the weeks before the examination. The first chapter details the way in which a psychiatric case should be presented. This is followed by a chapter discussing the signs and symptoms of psychiatric disorders, as the student must be able correctly to elicit these. There follow chapters dealing with the most important psychiatric disorders, beginning with those at the top of the diagnostic hierarchy, namely organic disorders, psychoactive substance use disorders and psychotic disorders. Finally, a glossary is provided; this should aid revision.
The classification of disorders used in this book is principally that of the 10th revision of the International Classification of Diseases of the World Health Organization, ICD-10. Occasionally, however, important subjects that are taught and examined at undergraduate level but do not appear in the ICD-10 classification of mental and behavioural disorders, such as premenstrual syndrome, have also been included.
The third edition of this book has once again been revised and updated. Newer drugs have been included, while some older ones have been removed. Drug names have been updated. Details of the mini mental-state examination, caffeine intoxication, child abuse, repetitive transcranial magnetic stimulation (rTMS), phototherapy, vagus nerve stimulation (VNS) and the McNaughten Rules are just seven examples of the new material which has been included in this edition. Accordingly, the sections on questions and answers for the final medicine examination have been increased somewhat.
Once again, I strongly recommend that the reader refer to the most up-to-date formulary available when learning about or prescribing individual drugs.

1. Case presentation

As with other branches of clinical medicine, it is important when assessing a patient to be able to take an accurate history and carry out an appropriate examination. The latter, in the case of psychiatry, includes not only a physical examination but also an examination of the patient's mental state. Together, the history and mental state examination allow the doctor to elicit the symptoms and signs of a psychiatric disorder, should one exist. In this chapter the headings under which the various parts of a case presentation should be given are outlined.


PSYCHIATRIC HISTORY
The reason for referral, the complaints and the history of the presenting illness should be given first.
The chronological sequence of each symptom should be determined at interview.
The psychiatric history should ideally be brought together from both the patient and sources of further information (see the section on Investigations later in this chapter).

Reason for referral
Briefly state how and why the patient was referred.

Complaints
These are the patient's complaints given in his or her own words. The length of time each complaint has lasted should also be given.

History of presenting illness
A chronological account should be given of the development of each symptom, together with any precipitating factors. Associated impairments should also be given. For example, for a depressive episode biological and cognitive symptoms of depression ( Ch. 6 ) should be included. The effects of the patient's condition on social functioning should be noted.

Family history
Give details of parents and siblings, including their:

• Current ages or ages at death

• Occupations

• Health

• Relationship with the patient.
The timing of parental separation and/or divorce, if relevant, should also be stated.
