There are far too many silent sufferers. Not because they don t yearn to reach out, but because they ve tried and found no one who cares. Richelle E. Goodrich
Silence can mean different things at different times to different people. It can be good; relished by many, craved by others who live amidst the hustle and bustle of their daily lives. There is silence by choice as well as unwanted silence. Silence can be an active energy with the power to get people to think and to act, it can help slow the mind down for quiet reflection. Its importance is also captured in songs like The Sounds of Silence .
Who are the silent sufferers ? Why do they exist? Of all the definitions of the word silence that speak to silent sufferers it is not the absence of sound or a refusal or failure to speak out which aligns but the condition or quality of being or keeping still and silent , which speaks volumes. Throughout the world there are many people who suffer in silence . Included in this group are the silent sufferers in maternity care, both the users and the providers.
This is the subject of The Roar Behind the Silence which seeks to raise awareness, explore the causes of distress in maternity care and ways in which we can address the issues, and find solutions - the what and how . This book is welcome. Infused with topics and words such as: relationships, compassion, dignity, kindness, resilience and learning - all key factors. Emphasis is also placed on the importance of the team of providers - midwives, obstetricians and others.
Silence can also be a powerful therapeutic and strategic tool. As Mark Twain, said The right word may be effective, but no word was ever as effective as a rightly timed pause . The Roar Behind the Silence sends a message that we are all in it together. And together we can alleviate the distresses. Our challenge is in working together - as service users and service providers - in getting this balance right.
Unlike the introductory quote there are many who care. Some of these voices can be heard loud and clear in the rightly timed The Roar Behind the Silence . A welcome and much needed roar. We urge readers to become champions adding voices to this roar. We would do well to be guided by these words of the Dalai Lama Love and compassion are necessities, not luxuries. Without them humanity cannot survive .

Having worked closely with midwives through my work with the International Confederation of Midwives, and my organisation, the Wellbeing Foundation Africa, I have been honoured to hear their stories and learn from their experiences. The closer I work with midwives, the stronger my belief that midwives are an integral part of the solution in reducing maternal mortality. The power and perseverance of midwives, and others involved in the safe delivery of newborns, creates an energy that is palpable. This book translates this energy into the written word.
The relationship between a mother and her midwife is rooted in a fundamental sense of mutual understanding and kindness. Caring, respectful and well-educated midwives result in more confident mothers, safer babies, and healthier families with better futures. Bringing together the voices of those committed to safeguarding women and children, Downe and Byrom have provided a platform for a much-needed discussion on respectful and compassionate care. Their goal to change the culture of midwifery from one of fear to one of love is a noble pursuit. After all, love - that echoes the enduring love between mother and child - is a far more compatible emotion for childbirth.
The writers explore how international, national and local systems and models of care can support and empower caregivers and women to engage in healthy, compassionate practices. There is an African proverb that says If you want to go fast, go alone. If you want to go far, go together. It is in this spirit that we should draw together our voices as women, mothers, midwives and caregivers. Through this collective call we can bring out the best practice and results in childbirth.
I thank Soo Downe and Sheena Byrom for their words, for taking the time to research, and for pulling together expertise from around the world to improve the lives of every woman and every child.

As editors of this book, our position is that the issue of kindness and compassion is absolutely fundamental to good quality maternity care. We came to this conclusion as midwives with many years experience in a range of clinical, research and management roles, working both in and out of hospital settings, and as a consequence of discussions with many people around the world about the state of maternity services. This book is the result of that concern. However, the contributing authors demonstrate that this is a far from simple issue.
