Foreword
For many years the identification and treatment of alcohol dependency have been seen by healthcare professionals as being firmly lodged within the agenda of mental health nurses, psychiatrists and the voluntary sector. As any GP, practice nurse, member of the emergency services or midwife will tell you, this is no longer true.
The misuse of alcohol is causing significant problems for modern society. Alcohol misuse is seen as a primary cause of childhood poverty and child abuse, of teenage antisocial behaviour, unplanned pregnancies, underweight and brain-damaged babies, chronic ill health, premature death, malnutrition, long-term mental health problems and aggressive behaviour. It is linked to obesity, family breakdown and unemployment and it can lead to drug abuse and criminal behaviour patterns. Its misuse affects not only the individual and his or her family but also wider society. And the cost of misuse to the police and NHS social services bill is staggering.
Every day, professionals are faced with patients and clients who are affected in one way or another by alcohol misuse and abuse.
One problem we all face is a lack of information. In order to provide the best support possible we all need to understand every aspect of the complex picture that surrounds those who seek help and support. Many of these people have complex histories, sometimes complicated by misrepresentation. They often forget to take prescribed medication, have abnormal nutritional patterns and are constantly aggressive, even to their best friends. They are damaged physically, psychologically and socially. They may have an underlying pathological condition such as diabetes, asthma or AIDS that is made worse by alcohol, and they may be homeless or poorly housed. Very often, textbooks offer a mere chapter or two on this complex and wide-ranging problem, and fail to address the ‘bigger picture’.
Colin Martin has brought together an excellent set of contributors to produce a truly comprehensive text that will ensure all interested professionals can not only update their knowledge but also better understand each other's roles and how best to help patients and clients through shared working to achieve a common goal of better health. This book is not just for people who see themselves as having a primary role in the field, but also for those – teachers, the police and family members – who live daily with its effects.

Acknowledgements
I would like to thank all those clinicians and academics who have generously contributed chapters to this book. The insights of these esteemed professionals in the field highlight the myriad of complex interacting factors that contribute to the significant issues surrounding and defining alcohol dependency. I would also like especially to thank two long-term research partners in the alcohol field – Professor Victor Preedy at King's College London and Dr Adrian Bonner at the University of Kent at Canterbury – for their friendship and collaboration over the past decade; our work together has always been a source of great pleasure to me.
I am also very grateful to Professor Dame Betty Kershaw at the University of Sheffield and Professor Cynthia McDougall O.B.E. at the University of York for their support and confidence in my academic work over the years, this book being one of the end-points.
My best friends – Phil Hutchins, Dr Mark Newberry, Dr Suzy Newberry, Julie Robbins and Peter Robbins – have also indirectly contributed to the production of this book by their enduring enthusiasm, interest and support of my work.
Thanks are also due to Mike Roberts at M&K Update for his support for the book and to Maria Hampshire for guiding it smoothly through the publication process.
Finally, I would like to acknowledge and thank the many clients and patients I have had the privilege of working with clinically over the past 20 years in the addictions field.
